Steps to follow: 

Ontario reporting > Ontario Student Records (OSR) > Office index card > GO 

  1. (1) Select by – do your selection 

  1. (2) Include student registered – confirm your selection 

  1. (3) Setup > Additional information – we recommend that you have the box checked for “Include titles and frames and you can decide to have two student per pages as well 

  1. (4) Print  

  1. (5) Go to the Bell > Preview > Print

Note that if information is missing in the form, verify with the Student records as the information should be also missing in it.
